CSCNH offers a variety of residential housing options for persons with developmental disabilities and/or traumatic brain injuries. Including our small community-based residential homes, which provide 24-hour staff coverage and necessary care in a homelike atmosphere. CSCNH also offers a Home Provider Service for clients ranging in ability from those who have significant cognitive/functional impairments to those who are quite independent. The certified and licensed home is fully accessible and provides 24 hour personalized care from a highly skilled staff who have received specialized training to meet the clients needs.Our Franklin Falls Farm residence is a beautiful old farm house located in Franklin, New Hampshire. It offers both the serenity of the country as well as the advantages of a community and town to its residents. All the residents are survivors of acquired brain injury. The program is designed for individuals who have completed traditional rehabilitation services, yet continue to need assistance for cognitive, vocational, social and independent living skills. An individualized plan is implemented for each client. This plan includes money management, menu planning, cooking skills, recreational pursuits, and long term planning. If needed, the plan will also focus on occupational, physical, and speech therapies as well as behavioral services. The goal is to assist each individual in reaching their maximum independence level and also in returning to their community. Pleasant Street - Concord
Located on Pleasant Street in Concord, New Hampshire this residence is a state certified home currently providing a home for three men, all of whom have complex medical needs such as Huntington's disease, cerebral palsy etc... The program specializes in addressing the medical, behavioral, nutritional, and personal safety needs of each resident. To assure individual attention, each resident has an individualized day program, which provides training in community living, self care and socialization skills. The residence is fully accessible and provides 24 hour personalized care from a staff who have received specialized training to meet the clients needs. Transportation of clients is provided, enabling the residents greater community involvement and access to family and friends.
